1)Cyanogen 7 mod or any other rom having T-mobile Theme chooser

2)LunarUI black-CM 7 theme

3)For the icons : metrostation icon package
steps:
*long press an icon on the destop and select edit
*click on the icon
*select Adw theme icon package
*select metrostation
*select the icon you want to replace the selected icon with
*select the colour,your done
*repeat for each icon you want to change
